I only had to knife him a few times to receive the achievement. Repeat this for a total of two times, then toss another flash and repeatedly knife him. Buy an MP5 and flash grenades, then toss a flash grenade at the Juggernaut, then shoot a whole clip of the MP5 at him. Join any survival game (tier one recommended for its easy difficulty) and progress to wave 10 for a Juggernaut spawn.
